2.   Fill in all fields:

   "child name"                                Deion                   (oldest child's name)
   "parent(s) first name"                  Melody, Dennis      (first name of parents/guardians)
   "grade/class"                              5th grade
   "email"                                 dennis@hotmail.com     (should be your login)
   
   "additional info"                          Dustin, 3rd Gr       (Add additional child's first name,  if applicable, followed by grade/class)
   
3. Congratulations, you are now registered and have access to the entire site! 

NOTE:  Please keep only ONE account per family (each family must have only one email/login account).  To request an exemption, please contact the website administrator.
